By signing this agreement, I understand that I have entered into a contract with Appleton Thorn Primary School Before and After School Link Club and will adhere to the guidelines, including payments.

By booking my child into Breakfast, After-School and/or Holiday Club I am giving permission for Starlings staff to care for my child for the duration of the session.
